ABSTRACT

Objetivo: Analisar se os cirurgiões-dentistas têm conhecimento das implicações judiciais embutidas na cirurgia estética de bichectomia. Métodos: Trata-se de um estudo descritivo no qual foi aplicado um questionário estruturado aos docentes cirurgiões-dentistas da Faculdade Maria Milza (FAMAM) que realizam bichectomia. Os dados do questionário foram organizados em forma de tabela, apresentando informações sociodemográficas e sobre o conhecimento deles em relação aos aspectos judicias embutidos neste tipo de cirurgia. Resultados: A amostra foi composta por 08 cirurgiões-dentistas, docentes da Faculdade Maria Milza. A maioria era do sexo masculino, com idade entre 29 a 39 anos, com tempo de docência menor igual a 5 anos e atuando em clínica privada. A maioria afirmou estar ciente das possíveis implicações judiciais e se previnir destas, no entanto, grande parte da amostra estudada não tinha conhecimento do Código Civil Brasileiro. Conclusão: A maioria dos dentistas não possuíam conhecimento sobre o Código Civil Brasileiro, mas sabem que podem responder processos movidos pelos pacientes. Frente a isso, eles estão se protegendo de possíveis implicações de ordem judicial relacionados a cirurgia de bichectomia.(AU)


Objective: To analyze whether dentists are aware of the judicial implications embedded in cosmetic surgery for bichectomy. Methods: This is a descriptive study in which a structured questionnaire was applied to the professors-dentists at Faculdade Maria Milza (FAMAM) who perform bichectomy. The questionnaire data were organized in form of a table, presenting sociodemographic information and their knowledge in relation to the judicial aspects embedded in this type of surgery. Results: The sample consisted of 08 dental surgeons, professors at Faculdade Maria Milza. Most were male, aged between 29 and 39 years old, with less than 5 years of teaching experience and working in a private clinic. The majority claimed to be aware of the possible legal implications and to prevent them, however, a large part of the sample studied was not aware of the Brazilian Civil Code. Conclusion: Most surgeons had no knowledge of the Brazilian Civil Code, but they know that they can respond to a lawsuit brought by patients. Faced with this, they are protecting themselves from possible implications of a court order related to bichectomy surgery.(AU)

ABSTRACT

Better understanding of dentists' decision-making about defective restorations is needed to close the evidence-practice gap (EPG). This study aimed to quantify the EPG about defective restorations and identify dentist factors associated with this EPG. Methods: 216 dentists from São Paulo State, Brazil, completed a questionnaire about three clinical case scenarios involving defective composite restorations with cementum-dentin margins (case 1) and enamel margins (case 2), and an amalgam (case 3) restoration. Dentists were asked what treatment, if any, they would recommend, including preventive treatment, polishing, re-surfacing, or repairing the restoration, or replacing the entire restoration. Replacing the entire restoration in any of these three scenarios was classified as inconsistent with the evidence, comprising an EPG. Bivariate analyses using Chi-square, ANOVA, or multiple comparison tests were performed (p<.05). Results: for defective composite restorations, 49% and 55% of dentists chose to replace the entire restoration for cases 1 and 2, respectively. Twenty-nine percent of dentists chose to replace the entire amalgam restoration. Dentists were significantly more likely to choose to replace the defective amalgam restoration than the composite restoration with a defect at the cementum-dentin margins or the enamel margins (both at p < .001). Female dentists were more likely to choose a conservative treatment than male dentists for cases 1 (p=.034) and 2 (p=.009). Dentists with a higher percentage of patients interested in individualized caries prevention were also more conservative in case 1 (p=.045). Conclusion: a substantial EPG regarding treatment decisions for defective restorations exists, especially for composite restorations. This study adds to the international evidence that an EPG exists in this clinical area and that global strategies need to be developed to close the gap

Objetivos: descrever as práticas e os conhecimentos de cirurgiões-dentistas (CDs) da atenção primária à saúde do município de Ribeirão Preto, SP, em relação à população em situação de rua (PSR), caracterizar o perfil dos cirurgiões-dentistas, avaliar a percepção dos profissionais em relação ao conhecimento da Política Na-cional para a População em Situação de Rua (PNPSR) e experiências com a PSR. Métodos: trata-se de um estudo transversal descritivo exploratório, utilizando questionário autoaplicável on-line. Os dados foram compilados pelo software Microsoft Excel versão 2010 e foi realizada análise descritiva dos dados. Resulta-dos: dos 29 CDs participantes, todos possuíam algum nível de pós-graduação, nunca receberam algum tipo de capacitação sobre a PSR (100%), mas têm interesse em recebê-la (82,7%). Consideram seu conhecimento sobre PSR como regular (41,4%) e se sentem parcialmente capacitados para identificar o uso abusivo de álcool (44,8%) e drogas (37,9%); 51,7% não têm contato com serviços de assistência social; 38% relataram experiências positivas durante os atendimentos à PSR, envolvendo a resolubilidade das urgências e a gratidão demonstrada pela PSR; 31% relataram experiências negativas, relacionadas ao atendimento sob efeito de substâncias, à agressividade e à não adesão ao tratamento. Apesar de possuírem conhecimento das caracte-rísticas sociodemográficas da PSR, 51,7% dos entrevistados desconhecem a existência da PNPSR. Conclusão: os resultados mostram que os CDs da atenção primária à saúde do município são qualificados, mas, em geral, possuem baixa familiaridade com as especificidades encontradas no atendimento da população e, em sua maioria, desconhecem a existência da PNPSR.(AU)


Objective: to describe the practices and knowledge of dentists in primary health care in the city of Ribeirão Preto-SP in relation to the Homeless Population (HP), to characterize the profile of dentists, and to assess the perception of professionals in relation to knowledge of the National Policy for the Homeless Population (NPHP) and their experiences with HP. Methods: this is an exploratory descriptive cross-sectional study, using an online self-administered questionnaire. Data were compiled using Microsoft Excel 2010 software and descriptive data analysis was performed. Results: The 29 participating dentists, who had some postgraduate level, never received any type of training on PSR (100%), but they were interested in receiving it (82.7%). They considered their knowledge of HP as regular (41.4%) and felt partially able to identify the abusive use of alcohol (44.8%) and drugs (37.9%). 51.7% have no contact with social assistance services. 38% reported positive experiences during consultations with the HP, involving the resolution of emergencies and the gratitude shown by the HP. 31% reported negative experiences related to care under the influence of substances, aggression and non-adherence to treatment. Despite having knowledge of the sociodemographic characteristics of PSR, 51.7% of respondents are unaware of the existence of NPHP. Conclusion: the results show that the primary health care dentists in the municipality are qualified, but, in general, are not familiar with the specificities found in the care of the population and, for the most part, are unaware of the existence of the National Policy for the Population in Street Situation.(AU)

ABSTRACT

Objetivo: identificar a percepção de cirurgiões-dentistas das redes pública e privada do município de Erechim, RS, sobre atenção em saúde a pacientes com necessidades especiais, avaliando conhecimento, preparo e limitações para o atendimento odontológico. Métodos: este estudo de caráter transversal ocorreu no período de março a agosto de 2019, por meio da aplicação de questionário próprio semiestruturado para cirurgiões-dentistas. Resultados: participaram da pesquisa 82 cirurgiões-dentistas, sendo a maioria (58,54%) atuantes na rede privada; 56,10% não cursaram uma disciplina específica em sua graduação sobre cuidados a pacientes com necessidades especiais. Em contrapartida, dos que cursaram, para a maioria, a disciplina era obrigatória e teórico-prática. Conclusão: apesar de somente 42,68% se sentirem muito bem ou bem preparados, somente 10,98% nunca realizaram atendimentos. Mesmo não tendo cursado uma disciplina específica em sua graduação, os cirurgiões-dentistas de Erechim, RS, realizam o atendimento e, quando necessário, o encaminhamento desses pacientes. Por manifestarem o interesse em se manterem atualizados sobre o tema, ações de educação continuada serão de fundamental importância.(AU)


Objective: assess dentists perception, about oral health care for patients with special needs, of public and private service of Erechim, RS, evaluating their knowledge, preparation and limitations for dental care. Methods: this cross-sectional research was conducted from March to August 2019, using questionnaires for dentists. Results: eighty-two dentists participated in the research, the majority (58.54%) are from the private service, 56.10% did not attend a specific discipline, in their graduation, about dental care for patients with special needs. In contrast, for those who attended, the discipline was compulsory and theoretical- -practical. Conclusion: although only 42.68% feel very well, or well prepared, only 10.98% never attended dental care. Even though they did not attend a specific discipline in their graduation, the dentists of Erechim, RS, perform dental care and, when necessary, they referral these patients. As they express their interest in keeping up to date on the topic, continuing education actions will be of fundamental importance.(AU)

Abstract Currently, the whole world is under the threat of the COVID-19 pandemic, and dentists are at high risk. The aim of this study was to investigate what kind of precautions Turkish dentists take in dental clinics during the COVID-19 pandemic. The study population consisted of dentists in Turkey. An online questionnaire (23 questions-57 items) was sent to a sample of Turkish dentists from March 16 to March 20, 2020. The questionnaire comprised a series of questions about dentists' demographic characteristics, their concerns, and the measures taken in dental clinics against COVID-19. This study included a total of 1,958 Turkish dentists. A total of 1,274 (65.1%) were general dentists, and 684 (34.9%) were specialists. Five hundred twenty-two (26.7%) dentists attended an informational meeting on COVID-19. Of these dentists, 69.8% were aware of COVID-19 according to their self-assessed knowledge scores. More than 90% of the dentists were concerned about themselves and their families. Only 12% of the dentists wore an N95 mask. Although Turkish dentists took some precautionary measures, they did not take enough precautions to protect themselves, the dental staff, and other patients from COVID-19. As the number of COVID-19 cases increased, the measures taken slightly increased in dental clinics as well. Dentists are strongly recommended to take maximum precautions in the clinical setting. The guidelines about the COVID-19 pandemic should be sent to all dentists by the regional and national dental associations.

Abstract Telehealth has been applied in the Unified Health System (SUS) as a tool for qualifying professionals and improving the healthcare provided to the population served by the system. The aim of the present study was to evaluate the impact of teleconsultations on the qualifications of the clinical approach of the oral health teams involved in primary health care (PHC) in the state of Rio Grande do Sul under the Telehealth Brazil Networks Programme. The sample population of the study was composed of 285 dentists and 132 oral health assistants belonging to the Family Health Teams. A chi-square test was used to investigate possible associations between the absolute number of teleconsultations performed and several independent variables, namely, gender, age, dental specialty, time since graduation, time working in the Family Health Strategy (FHS), employment status, type of training institution, familiarity with information technology, and work satisfaction at the health facility. The level of significance was p < 0.05. Use of the oral health teleconsultation service led to a reduction of more than 45% in the number of referrals to other levels of care. However, no significant association was found between the number of teleconsultations and the independent variables analysed. The use of teleconsultations proved to be a powerful tool for professional training, for increasing the effectiveness of PHC, and for improving the oral healthcare provided.

ABSTRACT

RESUMEN: La ansiedad puede ser un detonante de la disfunción temporomandibular (DTM). Se ha mencionado que el trastorno psicológico más frecuente en México es la "ansiedad" con 14,3 %. El objetivo es correlacionar y comparar los niveles de ansiedad y los grados de DTM en cirujanos dentistas residentes (CDR) de dos universidades mexicanas: la Facultad de Odontología de la Universidad de la Salle Bajío León, Guanajuato (FOULSB), y de la Facultad de Odontología de la Universidad Autónoma (FOUADY) de Yucatán México durante el periodo de septiembre 2017 a febrero 2018. El tipo de estudio fue correlacional, comparativo de cohorte transversal, siendo las variables: ansiedad (niveles según cuestionario de autoevaluación de ansiedad estado/rasgo STAI); signos y síntomas clínicos para el diagnóstico de DTM, (CDI/TTM Grupo I Desórdenes Musculares; Grupo II Desplazamientos discales; Grupo III Artralgia, osteoartritis). El tamaño de la muestra y criterios de inclusión en ambas universidades fueron: CDR en Odontología; que aceptaron participar en el estudio, mediante la firma del consentimiento informado y voluntario; hombres y mujeres; entre 25 a 34 años de edad. Criterios de exclusión: con tratamientos ortodónticos, quirúrgicos, enfermedades sistémicas o neurológicas y que sólo el día que se tomó la muestra presentaron otalgia o cefalea. El tipo de muestreo, por conveniencia. Se utilizó estadística descriptiva e inferencial. Los resultados: 40 CDR de la FOULSB, donde el 92,5 % (n=37) manifestaron tener niveles de ansiedad bajos, y el 7,5 % (n=3) moderado. El 65,8 % (n=26) presentó diagnóstico de DTM. De los 40 CDR de la FOUADY, 97,5 % (n=39) declararon tener niveles de ansiedad bajos, y el 2,5 % (n=1) obtuvo un nivel alto. El 67,5 % (n=27) presentaron signos y/o síntomas de DTM. La conclusión fue que no existieron diferencias estadísticamente significativas, ya que los CDR de ambas universidades presentaron porcentajes de DTM similares y también tuvieron niveles de ansiedad similares.


ABSTRACT: Anxiety can be a trigger for temporomandibular dysfunction (TMD). It has been mentioned that the most frequent psychological disorder in Mexico is "anxiety" with 14.3 %. The objective is to correlate and compare the levels of anxiety and the degrees of TMD in resident dentists (CDR) of two Mexican universities: the Faculty of Dentistry of the University of La Salle Bajío León, Guanajuato (FOULSB), and the Faculty of Dentistry. Dentistry of the Autonomous University (FOUADY) of Yucatan Mexico during the period from September 2017 to February 2018. The type of study was correlational, comparative crosssectional cohort, the variables being: anxiety (levels according to questionnaire self-assessment of anxiety state / trait STAI); clinical signs and symptoms for the diagnosis of TMD, (CDI / TTM Group I Muscle Disorders, Group II Displacement discs, Group III Arthralgia, osteoarthritis). The size of the sample and inclusion criteria in both universities were CDR in Dentistry; who agreed to participate in the study, by signing the informed and voluntary consent; men and women; between 25 to 34 years of age. Exclusion criteria: with orthodontic, surgical treatments, systemic or neurological diseases and that only the day the sample was taken showed otalgia or headache. The type of sampling, for convenience. Descriptive and inferential statistics were used. The results: 40 CDR of FOULSB, where 92.5 % (n = 37) reported having low anxiety levels, and 7.5 % (n = 3) moderate. Sixty-five point eight percent (n=26) presented diagnosis of TMD. Of the 40 CDRs of FOUADY, 97.5 % (n = 39) reported having low anxiety levels, and 2.5 % (n = 1) had a high level. Sixtyseven point five percent (n=27) presented signs and / or symptoms of TMD. The conclusion was that there were no statistically significant differences, since the CDRs of both universities presented similar DTM percentages and had similar levels of anxiety.

Abstract It was investigated the perception of service users in relation to the professional practice of dentists, based on gender differences. The Primary Care Assessment Tool (PCATool) was applied to 900 users of the Public Service in Curitiba, PR, Brazil. Sixty clinics were selected using random sampling, divided between conventional Primary Health Care (PHC) Units and PHC with Family Health System Units. The attributes of PHC that compound the PCATool were analyzed. A question was added about user preference regarding the gender of the dental professional, thus generating four dyads derived from user gender/dentist gender (FF, FM, MM, MF). The attributes were linked to the dyads by applying the independent sample t test. Using logistic regression, the dyads were linked to 23 factors relating to scaled-up care in PHC. Many users showed a clear preference for being attended by female dentists. Users who prefer to be cared for by women tend to better evaluate PHC on issues related to "active listening", while those who prefer to be cared for by male dentists highlight the attributes of "care integration" and "community guidance". In other factors and attributes studied, there is no difference between the care given by men or women, regardless the unit.


Resumo Investigou-se a percepção dos usuários do serviço quanto à prática profissional dos dentistas, com base em diferenças de gênero. O Primary Care Assessment Tool (PCATool) foi aplicado a 900 usuários do Serviço Público de Curitiba, PR, Brasil. Sessenta clínicas odontológicas foram selecionadas usando amostragem aleatória, divididas entre unidades de Atenção Primária à Saúde (APS) convencional e unidades de APS com Estratégia Saúde da Família. Os atributos da APS no PCATool foram analisados. Uma questão foi adicionada acerca da preferência do usuário em relação ao gênero do dentista, gerando quatro díades derivadas da combinação do gênero do usuário/gênero do dentista (FF, FM, MM, MF). Os atributos foram ligados às díades pelo teste t para amostras independentes. Por regressão logística, as díades foram relacionadas a 23 fatores referentes aos cuidados ampliados na APS. Muitos usuários mostraram preferência por dentistas mulheres. Esses tendem a avaliar melhor os cuidados de saúde primários quanto à "escuta ativa e qualificada", enquanto aqueles que preferem ser atendidos por homens destacam os atributos de "integração do cuidado" e "orientação comunitária". Em outros fatores e atributos estudados, não há diferença entre o atendimento prestado por homens ou mulheres, independentemente da unidade.

Resumo O abuso físico de crianças é definido como qualquer ferida não acidental ou omissão dos seus responsáveis que cause risco à sua integridade. Este estudo objetivou avaliar se profissionais de saúde percebem e denunciam o abuso físico em crianças/adolescentes. Um total de 62 profissionais de saúde (odontólogos, médicos e enfermeiros) de Diamantina/MG, que realizam atendimento a crianças e adolescentes, responderam a um questionário. As variáveis coletadas foram relacionadas à identificação de abuso; denúncia às autoridades e dificuldades para a realização da denúncia. Realizou-se análise de frequência, teste do qui-quadrado e análise de conteúdo. Todos os profissionais relataram ter identificado e denunciado a ocorrência de abuso físico em crianças/adolescentes. Associação significativa foi observada entre a especialidade do profissional e o reconhecimento de abuso, bem como a realização de denúncia às autoridades. As principais dificuldades apresentadas para denunciar os casos às autoridades foram: falta de conhecimento na identificação do abuso e em como realizar a denúncia. A maioria dos participantes gostaria de receber treinamento para identificação e denúncia de abuso. Os profissionais de saúde percebem o abuso infantil, sendo os médicos aqueles que mais denunciam os casos.


Abstract Physical abuse of children is defined as any non-accidental injury or omission of their caregivers that causes risk to the child's integrity. This study aimed to evaluate whether health professionals perceive and report physical abuse in children/adolescents. A total of 62 health professionals (dentists, physicians and nurses) from Diamantina/MG who attend to children and adolescents answered a questionnaire. The collected variables were related to the identification of abuse; denunciation to the authorities and difficulties in making the complaint. Frequency analysis, chi-square test and content analysis were performed. All professionals had identified and reported the occurrence of physical abuse in children/adolescents. Significant association was observed between the specialty of the professional and the recognition of abuse as well as denunciation to the authorities. The main difficulties presented to denounce the cases to the authorities were lack of knowledge in identifying the abuse and how to make the complaint. Most participants expressed that they would like to receive training in identifying and reporting abuse. Health professionals perceive child abuse, and physicians are the ones who most report the cases.

Objetivo: avaliar o conhecimento do cirurgião-dentista em relação a crianças e adolescentes que apresentaram alguma suspeita ou sinal de maus-tratos e verificar se esses profissionais relataram essa violência aos órgãos competentes. Materiais e método: aplicou-se um questionário com 238 cirurgiões-dentistas, incluindo 4 estados brasileiros, que previamente consentiram em participar da pesquisa. O questionário foi composto de perguntas abertas e fechadas acerca do assunto, bem como de figuras para identificação de maus-tratos. Os dados coletados foram digitados em um banco de dados no programa Microsoft Excel e analisados no programa Statistical Package for the Social Science (SPSS) 20.0. Resultados: a maioria dos cirurgiões-dentistas afirmou já ter realizado algum atendimento odontológico pediátrico em seu consultório e não ter reconhecido um caso suspeito de maus-tratos. Dos profissionais que já reconheceram um caso suspeito, 28,6% não souberam como proceder. Quando foi mostrada a cartilha de maus-tratos para todos os profissionais, incluindo aqueles que nunca atenderam crianças, 64,7% souberam reconhecer os sinais de maus-tratos demonstrados nas ilustrações. Conclusão: mesmo que os dentistas se considerem capazes de identificar casos de maus-tratos de crianças, ainda existe uma porcentagem de casos sem encaminhamentos às autoridades judiciais. É necessário que os profissionais recebam mais informações sobre a necessidade de estar alerta para a detecção de casos suspeitos de maus-tratos de crianças e adolescentes, bem como para o seu dever legal de relatar tais casos às autoridades. Objective: this study aimed to assess the knowledge of dentists on children and adolescents presenting some suspicion or sign of abuse and if they have reported such abuse to the competent agencies. Materials and method: a questionnaire was applied to 238 dentists who had consented to participate in the study; it included four Brazilian states. The questionnaire consisted of open and closed questions about the subject and it used figures to identify abuse. The data collected were entered into a database in the Excel software and analyzed in the Statistical Package for the Social Science (SPSS) 20.0. Results: most dentists reported having performed some pediatric dental care in their practice and have not identified a suspected case of abuse. From the dentists who had recognized a suspected case, 28.6% did not know how to proceed. When the booklet on abuse was shown to all professionals, including those who had never provided pediatric care, 64.7% were able to recognize the signs of abuse shown in the illustrations. Conclusion: even if dentists consider themselves capable of identifying cases of child abuse, there is a percentage of cases not referred to the legal authorities. Professionals need to be more informed about the need for awareness to detect suspected cases of child and adolescent abuse and to their legal duty to report such cases to the authorities. (AU)

Objetivo: avaliar o conhecimento de um grupo de cirurgiões-dentistas que trabalham em Atenção Primária à Saúde acerca de diagnósticos e condutas a serem tomadas diante de urgências em endodontia. Métodos: 24 cirurgiões-dentistas responderam um questionário estruturado, adaptado para o estudo. O instrumento abordava características socioeconômicas e o conhecimento dos cirurgiões-dentistas sobre diagnósticos e condutas perante patologias pulpares e periapicais. Os dados foram submetidos a análises estatísticas descritivas (medidas de variabilidade e de tendência central) e de associação entre os desfechos e as variáveis independentes, pelo Teste t Student e pelo teste de correlação Pearson, todos com α ≤ 0,05. Resultados: a quantidade de acertos dos participantes foi baixa nas perguntas selecionadas, as quais eram vinculadas aos conhecimentos de urgência em endodontia (27,3% ­ 68,2%). Essa baixa quantidade de acertos foi relacionada às variáveis sexo, idade e tempo de formado. Conclusão: o cirurgião-dentista deve estar preparado para lidar com urgências endodônticas desde a formação acadêmica. Considerando a baixa quantidade de acertos nas questões acerca de urgências endodônticas, atualizações constantes sobre o tema são necessárias, a fim de aprimorar o conhecimento dos profissionais e ampliar a resolutividade dos serviços. Objective: To assess the knowledge of a group of dentists working in Primary Health Care about the diagnostics and behaviors to be adopted to treat endodontic emergencies. Methods: 24 dentists answered a structured questionnaire adapted to the study. The instrument addressed socioeconomic characteristics and the knowledge of dentists about diagnostics and behaviors concerning pulpal and periapical pathologies. The data were subjected to analyses of descriptive statistics (measures of variability and central tendency) and of association between the outcomes and the independent variables using Student's t-test and Pearson correlation test. Significance was set at α ≤ 0.05. Results: The number of correct answers was low in the questions selected, which were linked to the knowledge of endodontic emergencies (27.3 - 68.2%). The low number of correct answers was related to the variables of sex, age, and time of academic training. Conclusion: Dentists should be prepared to deal with endodontic emergencies since their academic training. The low number of correct answers for questions about endodontic emergencies shows the need for constant updates on the subject, in order to improve the knowledge of the professionals and expand the resoluteness of services. Resumo O presente trabalho evidenciou o processo de trabalho do cirurgião-dentista (CD) no setor de saúde suplementar a partir da visão do profissional, sob a luz da Bioética de Intervenção. Foi realizado um estudo observacional-seccional do tipo inquérito circunscrito à região do Distrito Federal. Os dados foram coletados por meio de 108 questionários respondidos por CDs credenciados à duas modalidades de operadora: autogestão e odontologia de grupo, com a finalidade de conhecer a percepção e o grau de satisfação profissional diante do mercado de trabalho odontológico. A insatisfação maior por parte dos profissionais foi relacionada à remuneração dos trabalhos odontológicos pelas operadoras. Para a operadora de autogestão 1, 38,1% dos profissionais responderam que a remuneração era satisfatória, enquanto para a de autogestão 2 e odontologia de grupo, houve 100% de insatisfação. Outro dado encontrado foi que a operadora de odontologia de grupo restringiu os tratamentos selecionados aos pacientes de forma expressiva. Conclui-se que existe a perda de autonomia profissional, desvalorização dos ressarcimentos e precarização do trabalho odontológico na saúde suplementar, demonstrando conflitos éticos nessa relação de trabalho.


Abstract The present study highlighted the labour process of the dental surgeon (DS) in the private healthcare sector from the healthcare professional's perspective based on intervention bioethics. An observational, cross-sectional survey study was performed within the Federal District (Distrito Federal) region. Data were collected from 108 questionnaires completed by DSs affiliated with two types of private health insurers, self-insurance and group insurance, to assess job perception and the degree of job satisfaction in the dentistry market. The main source of dissatisfaction for healthcare professionals was related to the pay for dental procedures by insurers. For self-insurer 1, 38.1% healthcare professionals replied that the pay was satisfactory, whereas in self-insurance 2 and in the group insurance, 100% of healthcare professionals were dissatisfied. Another finding was that the group insurer considerably restricted elective treatments. In conclusion, loss of professional autonomy, depreciation of insurance claims and precarisation of dentistry occurs in the private healthcare sector, thus demonstrating the ethical conflicts in this relationship.

Objetivo: identificar a satisfação profissional de cirurgiões-dentistas atuantes na atenção básica do Sistema Único de Saúde em um município do sul do Brasil, que foi pioneiro na implantação de diretrizes municipais de saúde bucal. Sujeitos e método: para a coleta dos dados, foram utilizados componentes descritivos a partir de dados secundários obtidos por meio de um questionário aplicado para todos os cirurgiões-dentistas atuantes no referido serviço. Resultados: no total, 46 profissionais foram incluídos na amostra, sendo que 82,5% estavam satisfeitos em seu trabalho e, desses, o reconhecimento dos usuários do serviço foi o fator que os cirurgiões-dentistas mais associaram à felicidade. Já os fatores mais relacionados à infelicidade no trabalho são as críticas ao salário e os usuários não colaboradores; 97,8% acreditavam que seu trabalho fazia diferença na comunidade em que estavam inseridos. Conclusão: este estudo sugere que estabelecer diretrizes no processo de trabalho pode ter um impacto positivo na prática e na satisfação dos profissionais, e essas estratégias podem ser utilizadas como amparo para planejamento em gestão de saúde bucal de forma sistematizada. Objective: to identify the professional satisfaction of dentists working in the primary health care of the public health system, in a city in southern Brazil, which was a pioneer for implementing municipal oral health guidelines. Subjects and method: the data were collected by descriptive components from secondary data obtained through a questionnaire applied to all dentists working in the referred service. Results: a total of 46 professionals were included in the sample, whereas 82.5% were satisfied with their work, and the acknowledgment from service users was the factor that these professionals mostly associated with happiness. On the other hand, the factors most associated with unhappiness in the workplace are complaints of low wages and uncooperative patients. In addition, 97.8% of the professionals believed that their work made a difference in the community in which they were inserted. Conclusion: this study suggests that establishing guidelines in the work process may have a positive impact on the practice and the professional satisfaction and that these strategies may be used as a planning support for the systemic management of oral health. Abstract: This cross-sectional study aimed to evaluate the asynchronous dental teleconsulting of Telehealth Brazil Networks Program in Minas Gerais. Data were collected from secondary databases of the Clinical Hospital of the Universidade Federal de Minas Gerais (CH/UFMG) and Medical School of UFMG (MS/UFMG), from July 2015 to July 2017. The variables analyzed were telehealth center, sex and profession, date and time questions and answers, response time in days, whether the issue was duly addressed, whether the teleconsulting was attended, post status, professional's satisfaction, conduct adopted after teleconsulting, dental specialty area, and type of issues. Sociodemographic data from the Intermediate Geographical Regions (IGR) of Minas Gerais, including the Municipal Human Development Index (MHDI) and Gini Index were also collected. The results were analyzed using SPSS v.22.0. In total, 3,920 teleconsulting were recorded, with increasing demand in the study period. Most were requested by dentists (93.3%) and female professionals (78.6%), during office hours (78.6%), and related to general conducts (72.1%). Almost all were attended (99.7%), with a maximum response time of two days (75.0%). A low feedback on satisfaction and resolution of issues (missing data of 57.2% and 70.2%, respectively) was recorded. Only 5.2% had information on whether patients were referred. The most common inquires were of issues about Pathology/Stomatology specialties (19.0%) and Pharmacology/Anesthesiology (18.8%). The response occurred within the expected time, with prevalent issues in basic dental areas and general conducts. Feedback from professionals must be encouraged to allow a better analysis and improvements of the program.

ABSTRACT

Abstract The aim of this study was to understand the performance of dentists in public health about mouth cancer. A cross-sectional study was performed whit 121 dentists from Fortaleza Health Department, interviewed through a semi-structured questionnaire. The study did not identify statistically significant difference regarding the ability to perform biopsy by gender, year of graduation, expertise or time of public service. Only 22 professionals (18.2%) reported being able to perform biopsies and 13 (10.7%) would do so at the Family Health Unit. The reference and counter-reference system, by including another stage in patient care, increase the possibility of absenteeism from patient to health service. Although clearly identify the relevance of oral cancer mortality in the population, the vast majority of dentists is not equipped to perform the biopsy as a routine activity. Whereas the vast majority of biopsies of the oral tissues is performed as an outpatient, low technological complexity to perform the procedure and the effectiveness of biopsy in the early diagnosis of oral cancer, it is essential to hold this procedure in primary care, what can effectively contribute to decreasing mortality from oral cancer.


Resumo O objetivo do presente estudo foi analisar a atuação dos cirurgiões-dentistas da rede pública de saúde frente ao câncer de boca. Foi realizado estudo transversal no qual foram entrevistados 121 dentistas a partir de questionário semiestruturado. O estudo não identificou diferença estatisticamente significativa em relação à capacidade de realizar biópsia por sexo, ano de graduação, especialização ou tempo de serviço público. Apenas 22 profissionais (18,2%) relataram ser capazes de realizar biópsias, e 13 (10,7%) poderiam fazê-la na Unidade de Saúde da Família. O sistema de referência e contrarreferência, por meio da inclusão de mais de uma etapa na assistência, aumenta a possibilidade de absenteísmo do paciente ao serviço de saúde. Embora identifique claramente a relevância da mortalidade do câncer bucal na população, a grande maioria dos dentistas não está preparada para realizar a biópsia como uma atividade de rotina. Considerando que a grande maioria das biópsias dos tecidos orais é realizada em ambulatório, a baixa complexidade tecnológica para realizar o procedimento e sua eficácia para o diagnóstico precoce do câncer de boca, é essencial a realização deste procedimento na atenção primária, o que pode efetivamente contribuir para a diminuição da mortalidade por câncer oral.

Resumo Objetivou-se confrontar a visão de usuários e de profissionais da saúde acerca da satisfação com o serviço público odontológico. Realizou-se entrevista com usuários, profissionais considerados diretamente (ESB) e indiretamente (ACS) envolvidos com a saúde bucal. As variáveis foram avaliadas individualmente e agrupadas em domínios. Para análise criou-se um escore e aplicou-se o teste Kruskal-Wallis. Os entrevistados avaliaram positivamente a qualidade dos serviços odontológicos. Os ACS demonstraram satisfação menor na comparação com usuários quanto à satisfação geral, nos domínios Estrutura Física e Cuidados Médicos, e nas variáveis: qualidade do atendimento clínico; orientação ao paciente após tratamento realizado na especialidade; e período de atendimento odontológico. Os profissionais da ESB expuseram satisfação maior do que a dos usuários no domínio Cuidados Médicos e nas variáveis relacionadas à prestação de orientação, esclarecimento de dúvidas, resolubilidade na atenção e agendamento de consulta. Conclui-se que os profissionais da ESB demonstraram alto grau de similaridade com a satisfação exposta pelos usuários, ainda que positivamente ampliado, e os ACS expuseram opiniões discrepantes, de forma mais crítica em relação aos usuários.


Abstract The objective was to confront the view of users and health professionals about the satisfaction with the public dental service. Interviews were conducted with users, professionals considered directly (ESB) and indirectly (ACS) involved in oral health. Variables were evaluated individually and grouped into domains. A score was created for the analysis and the Kruskal-Wallis test was applied. The respondents positively assessed the quality of the dental services. The ACS demonstrated less satisfaction compared to users regarding general satisfaction in the Physical Structure and Medical Care domains and the variables: quality of clinical care; guidance to the patient after treatment performed in the specialty; and period of dental care. The ESB professionals expressed greater satisfaction than the users in the Medical Care domain and in the variables related to the provision of guidance, answering questions, attention solvability and consultation scheduling. It is concluded that the ESB professionals demonstrated a high degree of similarity to the satisfaction displayed by users, although positively expanded, and the ACS expressed more critically discrepant opinions in relation to users.

ABSTRACT

Abstract Questionnaires for the assessment of knowledge and self-perception can be useful to diagnose what a dentist knows about management and administration. The aim of the present study was to create and validate the Dental Management Survey Brazil (DMS-BR) scale, based on meetings with experts in the field. After having elaborated the first version, 10 audits were performed in dental offices in order to produce the final version, which included nine dimensions: location, patient, finance, marketing, competition, quality, staff, career, and productivity. The accuracy of the instrument was measured by intrarater and interrater reliability. In the validation phase, 247 Brazilian dentists answered a web-based questionnaire. The data were processed using Stata 13.0 and the significance level was set at 95%. The instrument had intrarater and interrater reliability (ICC-0.93 and 0.94). The overall average of respondents for the DMS-BR scale was 3.77 (SD = 0.45). Skewness and kurtosis were below absolute values 3 and 7, respectively. Internal validity measured by Cronbach’s alpha was 0.925 and the correlation of each dimension with the final result of the DMS-BR ranged between 0.606 and 0.810. Correlation with the job satisfaction scale was 0.661. The SEM data ranged between 0.80 and 0.56. The questionnaire presented satisfactory indicators of dentists’ self-perception about management and administration activities.

O trabalho assume um papel cada vez mais central na vida das pessoas, assim, é previsível que modelos modernos de tecnologias de gestão se ocupem também da qualidade de vida no trabalho (QVT) e dos fatores que incorporam a satisfação do indivíduo em sua atividade laboral e na humanização das situações relacionadas ao trabalho humano. Alguns estudiosos aventam a possibilidade do estresse contínuo no ambiente de trabalho acarretar prejuízos à integridade física, psicológica e ao convívio social e familiar dos profissionais. Assim à medida que o trabalho evolui e se torna mais dependente da técnica, ocorre um maior desgaste físico, psíquico e mental nos trabalhadores, isto acontece em função das exigências impostas pelas ocupações de ocupacionais e aumenta de forma assustadora o número de acidentes e doenças ocupacionais. Desta forma, a prática da odontologia predispõe seus profissionais a desenvolverem, sobretudo doenças do sistema osteomuscular, levando a incapacitação para o trabalho se não forem adotados princípios de ergonomia aplicados ao serviço. De maneira geral, no ambiente profissional há uma série de estímulos que podem contribuir significativamente para o surgimento de um conjunto de sintomas que são classicamente associados ao estresse laboral ou ocupacional. O objetivo do presente estudo consistiu em investigar os possíveis sintomas que acometem os cirurgiões dentistas da rede pública e da rede privada do município de Bauru/SP e sua relação com a atividade laboral, por meio do questionário Nórdico de Sintomas Osteomusculares (QNSO), avaliando também a presença da perda muscular manual utilizando um dinamômetro e assim investigar o impacto que esses possíveis fatores de risco causam na qualidade de vida desses cirurgiões dentistas utilizando o questionário QWLQ-Bref (Quality of Working Life Questionnaire Questionário de Qualidade de Vida no Trabalho). Do total de 1066 cirurgiões dentistas regularmente inscritos no Conselho Regional de Odontologia de Bauru (CROSP), apenas 128 (100,00%) participaram desta pesquisa, onde 64 cirurgiões dentistas pertencem à rede pública e 64 cirurgiões dentistas pertencem à rede privada. Tendo em vista a relevância sobre o tema Sintomas de LER/DORT, assim como os resultados também mostram que ao comparar a rede pública com a rede privada pôde-se perceber que na rede privada é maior é a frequência dos relatos de queixa de dor, desconforto ou dormência.(AU)


Work plays an increasingly central role in people's lives, so it is expected that modern models of management technologies will also focus on quality of life at work (QWL) and factors that incorporate the satisfaction of the individual in his work activity And the humanization of situations related to human work. Some scholars point out the possibility of continuous stress in the work environment causing damages to the physical and psychological integrity and to the social and familiar life of the professionals. Thus, as the work evolves and becomes more dependent on the technique, there is a greater physical, mental and mental exhaustion in the workers, this happens in function of the exigencies imposed by occupations of occupational and increases in a frightening way the number of accidents and occupational diseases. In this way, the practice of dentistry predisposes its professionals to develop, especially diseases of the musculoskeletal system, leading to incapacitation for work if principles of ergonomics applied to the service are not adopted. In general, in the professional environment there are a number of stimuli that may contribute significantly to the appearance of a set of symptoms that are classically associated with occupational or occupational stress. The objective of the present study was to investigate the possible symptoms that affect the dental surgeons of the public network and the private network of the city of Bauru / SP and its relation with the work activity, through the Nordic Questionnaire of Osteomuscular Symptoms (QNSO), evaluating Also the presence of manual muscle loss using a dynamometer and thus investigate the impact that these possible risk factors cause in the quality of life of these dental surgeons using the QWLQ-Bref questionnaire (Quality of Working Life Questionnaire). Abstract: Poor description of oral lesions jeopardize the prioritization of appointments in Oral Medicine. The present study investigated whether the use of support tools changes the quality of descriptions focusing on health care prioritization. Two oral lesions (A and B) were described by 64 dental students and 48 dentists using three methods: (a) without support tools (free); (b) using the oral examination form from the Specialties Manual in Oral Health/Brazilian Ministry of Health (SMOH form); and (c) using the OralDESC guideline. The descriptions were compared with a gold standard and percentage of agreement was analyzed by the Kruskal-Wallis and Dunn's tests (p<0.05). Descriptions with the OralDESC presented higher information quality. Considering items essential for prioritization, the OralDESC demonstrated better performance for lesion A; for lesion B, free descriptions and descriptions using the OralDESC were of higher quality than those using SMOH form. Therefore, the OralDESC offered greater support for the description of oral lesions for health care prioritization in Oral Medicine.

ABSTRACT

Objetivo Caracterizar a los odontólogos que ejercen la profesión en Chile, inscritos en el Registro Nacional de Prestadores Individuales. Material y método Estudio descriptivo, transversal, efectuado con la base de datos del Registro Nacional de Prestadores Individuales de la Superintendencia de Salud, solicitada vía Ley de Transparencia. Resultados A fecha de septiembre de 2014, se encontraban registrados 15.901 odontólogos titulados, desde 1945 en adelante. Su promedio de edad es 42,9 ± 11,46 años. El 54% son varones, y el 46%, mujeres. El 89,6% de los odontólogos chilenos estudió en Chile, el 73% de ellos en universidades tradicionales. Un 7,3% de los profesionales registrados son extranjeros, cuya gran mayoría estudió fuera del país. Para ellos, la vía más común de revalidación de título es mediante convenios con el Ministerio de Relaciones Exteriores. La universidad tradicional con más titulados es la Universidad de Chile, mientras que la Universidad Mayor es la privada con mayor egreso. En el último quinquenio, la cantidad total de titulados de universidades privadas superó al total de egresados de universidades tradicionales. Existen 4.339 registros de especialidad, un 49% de ellos certificado en el quinquenio 2009-2013. La especialidad más frecuente es Ortodoncia, con un 19,5% del total. La vía de certificación más frecuente es el posgrado universitario. Conclusiones Esta investigación constituye uno de los escasos estudios con información oficial y no especulativa sobre odontólogos en Chile. Consideramos que el Registro Nacional de Prestadores Individuales debe mejorar el acceso y registro de sus datos, para así caracterizar de forma adecuada, tanto demográfica como académicamente, a estos profesionales.


Objective To characterise dentists who practice in Chile, enrolled in the National Register of Individual Providers. Material and method Descriptive and cross-sectional study conducted with the database of the National Register of Individual Providers of the Superintendency of Health, requested through the Transparency Law. Results In September 2014, there were 15,901 registered dentists, of whom 54% were male, and graduated from 1945 onwards. Their mean age was 42.9 ±11.46 years. The large majority (89.6%) of Chilean dentists studied in Chile, 73% of them in traditional universities, and 7.3% of registered professionals are foreigners, most of whom studied abroad. For them, the most common route of degree equivalency is through agreements with the Chilean Ministry of Foreign Affairs. The traditional university with most graduates is the University of Chile, while Universidad Mayor is the private university with most graduates. In the last 5 years, the total number of graduates from private universities surpassed the total of graduates from traditional universities. There are 4,339 specialty records in the register, 49% of them certified in the period 2009-2013. The most common specialty is orthodontics (19.5% of the total). The most common route of certification is a university degree. Conclusions This study is one of the few with official, non-speculative information about dentists in Chile. We believe that the National Register of Individual Providers should improve access and registration of their data, with the purpose of properly characterising these professionals, demographically and academically.
